چکیده

Abstract Aim We aim to report the outcomes of two-stage revision surgery eradicate periprosthetic joint infection (PJI) endoprostheses in orthopaedic oncology. Method A retrospective review 44 patients who underwent PJI between 1999 and 2018 was conducted at a single tertiary oncology centre. The mean age 36.1 years (12–78). sites prostheses were distal femur 50%, proximal 11.4%, tibia 36.4% total with replacement 2%. follow-up period 6.9 (range 0.4–19 years). Results overall rate eradication for 95.2% 2 (95% CI 89–100%) 79% 67–93%) 5 years. Amputation free survival over 100% 88% 78–99.8%). There no difference incidence reinfection recurrence amputation silver-coated implants. Coagulase-negative staphylococcus most common microorganism, isolated 21 patients. Infection successfully eradicated 54.5%. Polymicrobial recorded 18%. Multi-drug resistance (MDR) organisms 48% cohort. Conclusions MDR polymicrobial are oncological lower clearance rates than primary arthroplasty group. increased attempts limb salvage treat may explain infection-free compared previous series same centre, which reported higher as their initial management.

Two-stage revision for prosthetic joint infection: predictors of outcome and the role of reimplantation microbiology

OBJECTIVES We describe rates of success for two-stage revision of prosthetic joint infection (PJI), including data on reimplantation microbiology. METHODS We retrospectively collected data from all the cases of PJI that were managed with two-stage revision over a 4 year period. Patients were managed with an antibiotic-free period before reimplantation, in order to confirm, clinically and micr...

Two-Stage Total Knee Arthroplasty for Prosthetic Joint Infection

PURPOSE This retrospective review was conducted to identify prognostic factors for two-stage reimplantation for infected total knee arthroplasty (TKA) and the rate of reinfection following revision TKA. MATERIALS AND METHODS Out of 88 patients diagnosed with post-TKA infection between 1998 and 2011, 76 underwent two-stage reimplantation and were reviewed in this study. The 76 patients were di...

Managing uncertainty - a qualitative study of surgeons’ decision-making for one-stage and two-stage revision surgery for prosthetic hip joint infection

BACKGROUND Approximately 88,000 primary hip replacements are performed in England and Wales each year. Around 1% go on to develop deep prosthetic joint infection. Between one-stage and two-stage revision arthroplasty best treatment options remain unclear. Our aims were to characterise consultant orthopaedic surgeons' decisions about performing either one-stage or two-stage revision surgery for ...

Revision for prosthetic joint infection following hip arthroplasty

OBJECTIVES We used the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man (NJR) to investigate the risk of revision due to prosthetic joint infection (PJI) for patients undergoing primary and revision hip arthroplasty, the changes in risk over time, and the overall burden created by PJI. METHODS We analysed revision total hip arthroplasties (THAs) performed due t...

Titanium-copper-nitride coated spacers for two-stage revision of infected total hip endoprostheses

Within the first two years after total hip arthroplasty implant-associated infection has become the second most common reason for a revision surgery. Two-stage implant exchange is frequently conducted using temporary spacers made of antibiotic-loaded cement in order to prevent a bacterial colonization on the spacer. Avoiding several disadvantages of cement spacers, a conventional hemi-endoprost...
